About this program

Continuum of Care is a transitional housing program. Intensive case management is provided to these households on a weekly basis, often in the home. For this program, CCA leases two rental units for these clients. We provide utility payments as well as some other funding toward supportive services such as medical appointment co-pays or money to pay for identification papers. These clients may remain in this housing for six
